Do you accept insurance?
No, unfortunately we are considered an out of network provider, therefore we do not work directly with any insurance. Office visits, medications, supplements etc. will be self-pay. Although for blood work, we are able to send insurance to the lab and the lab will be able to run your insurance to evaluate if you qualify for coverage.
How soon can I book an appointment?
Just give us a call at (480) 557-9095! Our patients are our priority, we will work with you to find an appointment that fits best with your schedule.
What happens on the first visit?
Prior to your first visit, you will be required to fill out new patient paperwork and consent forms, this is available on our website under the “contact us” tab.
